Pet Shop Puppy Request Form

Please review your paperwork carefully and include all information available to you.  Do not enter "do not know" for a breeder's name.  We MUST have a breeder's name and any reports received that do not have this information will be deleted with no further contact to you.  MAKE SURE your email address is correct!  WE ONLY research pet store puppies! 

A pet store is defined as any person or establishment that TAKES POSSESSION of puppies that they DID NOT breed, and sells them to the public.  So someone in NYC that advertises puppies in the newspaper, but they did not breed the puppies themselves, would be considered a pet store.  A website, like puppyfind.com, does NOT take possession of the puppies, so they would not be classified as a pet store.

Please be very specific, including any coughing, running nose, sneezing, parasites, kennel cough, etc. You can also insert comments here to better explain answers from above.
